I've long resisted having a missed appointment/late cancellation fee but, because of the increased numbers of patients who either no-show for their visit or call with last minute cancellation, I've felt it necessary to initiate a new policy that will hopefully help us address this problem. We understand that life happens and and exceptions can be made depending on circumstances but fees may be charged for late cancellations/missed appointments. Please download the attached form. You will need to sign it and return it to our office to continue as a patient at our practice.
